This notice explains how inquiries through this website are handled by Native Tucson Notary & Insurance LLC.
Information you provide
The inquiry form asks for your name, email, phone number, service requested, general service-area information, contact-time preference and message. This information is used to respond to your request and arrange services. The website does not offer customer accounts, take payments or accept document uploads.
Keep inquiries to scheduling details
Do not submit Social Security numbers, dates of birth, identification documents, financial account information, fingerprints, medical information or confidential legal documents through this form. Contact Kathy to arrange an appropriate way to exchange any information required for your appointment.
Form processing and email
The form sends information over HTTPS to FormSubmit, a third-party service that forwards inquiries to Kathy’s business email. FormSubmit states that it retains submissions for 30 days. Email delivery and storage are also handled by email providers. This inquiry form is not a secure document portal.

Google service-area map
The interactive Google map loads automatically on pages showing the service area. This connects your browser to Google, which receives connection information such as your IP address and may use cookies or your existing Google sign-in. The map does not sign you into the map creator’s account. The service communities are also listed as text alongside the map.
Hosting and external links
Hosting and security providers may process technical request information and use security cookies to operate and protect the website. Links to social networks, reviews and business profiles lead to separate services with their own privacy practices.
